NATIONAL CHAMPIONSHIP ENTRY FORM
WORLD SERIES ENTRY FEE:
14 years old and Under = $600.00 (+$25.00 if not registered
w/CABA)
15 years old and up= $700.00 (+$25.00 if not registered w/CABA or
+40.00 for Nations)
Note: Add $75.00 at large berth fee, if you did not qualify through
league or tournament

CODE OF UNDERSTANDING:
- All entries are subject to approval of the National Championship Director or the CABA National Executive Director.
- The Directors are under no obligation to accept any entry that arrives after the bracket is drawn.
- Any team that submits a bad check for payment of entry fee or motel bills will be subject to disbarment.
- All teams will be ready to play on opening day of the tournament through the last day of the tournament.
- All teams MUST stay in Tournament sponsored hotels, or will be subject to disbarment.
